Registration

Thank you for your interest in the International Society of Hip Arthroscopy 2^nd Annual Scientific Meeting in Cancun, Mexico. For Registration categories and fees, as well as passport information, please see below.

Registration Fees

* Payment for 2010-2011 ISHA Member Dues is required in advance of meeting registration to receive Member meeting registration discount. Per ISHA executive offices, refunds for member discount cannot be given. For questions regarding ISHA Membership, please visit the ISHA website.

** Resident/Fellow Registrants must provide proof of status or a letter from Residency/Fellowship Program Chair when picking up badge onsite.  Those who are unable to provide proof of status will be asked to pay the full registration price.

Methods of Registration

Attendees may register by using one of three methods:

• Online
• Mail
• Fax

All pre-registered ISHA attendees will be emailed confirmations.  If you have not received a confirmation within three weeks of registering, please contact the Course Office.

Registering Online

Attendees may register for the International Society of Hip Arthroscopy 2^nd Annual Scientific Meeting online using their credit card (Visa, American Express and MasterCard). To register, please log into ISHA and complete the registration form and payment. If you are unable to register online, please contact the Course Office.

To pre-register via mail or fax, please complete the registration form.  Make sure to include all contact information (email address, mailing address, telephone and fax) to ensure receipt of your Registration Confirmation Email.  All registrations MUST be accompanied by payment.  Registrations without payment will not be processed.

Registering by Mail

Attendees may mail their completed registration form with credit card information or check to the Course Office. Please note all checks must be in US Dollars from a US Bank. Checks should be made out to MCJ Consulting, LLC.

CANCELLATION

Refunds will be subject to a US $100.00 processing fee, and may take up to four weeks to be processed. Requests for refunds must be received in the Course Office by September 15, 2010. No refunds will be issued after September 15.

HIP ARTHROSCOPY MODEL LAB

The International Society for Hip Arthroscopy is pleased to offer a Hip Arthroscopy Model Lab to our Annual Scientific Meeting participants!

The Model Lab will be offered in two sessions over the course of the meeting. Registration for the Model Lab is offered at an additional charge of US $125. Model Labs will be held on Friday, October 8, from 4:30 pm – 6:00 pm and Saturday, October 9, from 7:00 am – 8:30 am. Please note, registration is on a first-come, first-serve basis. PASSPORT AND VISA REQUIREMENTS

A valid Passport is required to enter Mexico. Participants from certain countries may be required to obtain a visa to enter Mexico. If you are uncertain to your requirement please consult your nearest Mexican Embassy/ Consulate or visit the website of Ministry of Foreign Affairs of Mexico.
