Fellows may register online using a credit card (Visa, American Express and MasterCard). To register please complete the online registration form including email address, mailing address, telephone and fax. All registrations MUST be accompanied by payment. Registrations without payment cannot be processed.
REGISTERING BY MAIL
Attendees may mail their completed registration form with credit card information or check to the Course Office. Please note all checks must be in US Dollars from a US Bank. Checks should be made out to MCJ Consulting, LLC.

CANCELLATION
Refunds will be subject to a US $50.00 processing fee, and may take up to four weeks to be processed. Requests for refunds must be received in writing by the Course Office by December 15, 2011. No refunds will be issued after December 15.
GROUP REGISTRATIONS
Fellowship Programs wishing to register multiple Fellows may choose to register them as a group. Please contact MCJ Consulting directly for further information regarding Group Registrations.
SURGICAL SKILLS WORKSHOPS: ARTICULAR CARTILAGE REPAIR AND ACI
Surgical Skills workshop sessions are included in the registration fee and will be held in two sessions: Friday, January 27 from 1:00 – 4:00 pm and Saturday January 28 from 3:00 – 6:00 pm. Fellows will be assigned to the sessions based on travel requirements. Space is limited. Enrollment in the Workshops is on a first-come, first-served basis.
OPTIONAL SMALL GROUP DEMONSTRATIONS OF HIP ARTHROSCOPY
An optional Lecture and Workshop session will be held on Sunday, January 29 for interested fellows. This session requires an additional registration fee of $50.00 . The session will include lectures and cadaver demonstrations on hip arthroscopy by expert surgeons. Please note, registration is on a first-come, first-served basis.
